2025 Football Season Agreement and Consent

The undersigned hereby grants permission for the above named youth to participate in any and all activities of the “York Seaford Flacons”. I assume all risk incidentals to such participation, including transportation to and from all activities. I hereby waive, release, absolve, indemnify and agree to hold harmless PYFCO, York Seaford Flacons, its’ league officials, respective coaches, other players, parents/ guardians, sponsors, participants, volunteers and any other persons transporting my child. I further grant permission for emergency first aid to be given to the minor child in case of injury. Furthermore, I grant permission for my minor child to be taken to the emergency room of a nearby hospital, and the hospital and its medical staff have my authorization to provide treatment which a physician deems necessary for the well-being of my minor child.

Volunteering: By submitting this registration, I understand that every parent is required to support the league by working in the concession stand for 1 sixty minute shifts or assist on the field during game day as well as trash pick up, etc. per child. There is no opt out fee for volunteering; everyone is expected to do their part. Unless extenuating circumstances and after final review by "York Seaford Flacons Board”, any parent that does not fulfill this obligation will have their child benched for 1 game and/or up until that obligation is fulfilled.

Equipment Issued: I understand that I am responsible for any equipment or uniforms received during the season and agree to return all equipment at the end of the season or I will be charged for the cost of the equipment or uniform.

Photo release statement: I also hereby consent, without further consideration or compensation, to the use (full/partial) of all photos, audios or videos made of my child or family members during York Seaford Flacons sporting events, for the purposes of event documentation, promotional materials or web content . Further, I release York Seaford Flacons and its volunteers from any liability, which may arise from the use of those photos.
